On an electric tank that is most often one of the two heating elements or its thermostat - typically the upper one when the water is lukewarm from the start, the lower one when it runs hot then cold. On a gas tank it points at the burner assembly, the thermocouple or the gas control valve. All of it needs the supply isolated before testing.

Sediment on the tank floor, with water boiling underneath it. Flushing the tank on the schedule in the manual is the preventive fix; heavy long-standing sediment reduces both efficiency and tank life, and at that stage we will tell you frankly where the tank is in its life.

Treat it as urgent. Check whether it is condensation, a fitting, or the temperature-and-pressure relief discharge line before assuming the worst, but a tank leaking from the body itself cannot be repaired and needs replacing.
